**CHC residents have Monday afternoon continuity clinic

Learning Objectives

  1. Participate in a family meeting to address goals of care.
  2. Assess pain and develop an initial treatment plan.
  3. Correctly perform opioid conversions (oral, IV, PCA)
  4. Assess non-pain symptoms and develop an initial treatment plan.
  5. List eligibility requirements for the Medicare Hospice Benefit
  6. List the covered services of the Medicare Hospice Benefit
  7. Identify disease specific factors indicating a prognosis of less than 6 months for the common hospice diagnoses: cancer, dementia, chf

Resident Milestones

  1. Demonstrate a commitment to relieve pain and suffering
  2. Provide support (physical, psychological, social and spiritual) for dying patients and their families
  3. Provide internal medicine consultation for patients with more complex clinical problems requiring detailed risk assessment
  4. Engage in collaborative communication with all members of the health care team
