Learning Objectives

  1. Describe how care in the nursing home setting differs from acute hospital care and outpatient care
  2. Describe how subacute rehabilitation and long-term custodial care in the nursing home is financed
  3. List potential sources of medical error that occur during care transitions from hospital to nursing home

Resident Milestones

  1. Effectively communicate with other caregivers in order to maintain appropriate continuity during transitions of care
  2. Manage and coordinate care and care transitions across multiple delivery systems, including ambulatory, subacute, acute, rehabiliation and skilled nursing
