Learning Objectives

  1. Perform a functional assessment on an ambulatory older adult.
  2. Perform a falls screen on an ambulatory older adult.
  3. Perform screening for urinary incontinence on an ambulatory older adult.
  4. Perform cognitive screening on an ambulatory older adult.
  5. Practice incorporating remaining life expectancy and patient preferences into screening and treatment decisions.
  6. Practice patient education in patients with low health literacy and/or impaired cognition.
  7. Perform a medication review in an older adult patient.

Resident Milestones

  1. Recognize disease presentations that deviate from common patterns and that require complex decision making
  2. Integrate clinical evidence, clinical context, and patient preferences into decision-making
  3. Utilize patient centered educational strategies

Logistics

  1. Contact: Individual provider based on schedule
  2. Location: Outpatient Senior Health Center at South Shore (7101 S. Exchange Ave.). Click here for map.
  3. Hours: 8 AM for morning clinics, 12:30PM for afternoon clinics, unless otherwise noted on schedule. (If you're scheduled to be in South Shore Senior Clinic all day, it's difficult to get to morning report/noon conference on campus, but we will leave this decision up to you, especially if there is a particular conference you want to attend. If you are at South Shore Senior Clinic for half a day, we encourage you to attend morning report/noon conference before/after you join geriatrics clinic. Please let your attending know if you need to come to clinic late or leave clinic early for this reason. )

Assignments
